Documentation

We all know surgery is the easy part. The hard work is done behind the front desk in order to have our patients prepared and on time for each and every appointment. With the high volume surgery model that we bring with us, it is easy for your staff to feel overwhelmed. However, we are here to lighten that burden and train your staff on the efficiencies.

Along with proper training we have our own surgery intake portal allowing patients to directly access their pre-op medical assessments from home and relieving the front desk of extra paperwork. Please see our page on Surgery Forms for more information on our patient portal.

We do not expect the host office to provide any new documentation. If they have a current system in place for surgery, we will do our best to work within it using our tried and true referral process.  We have you covered on documentation.

What we can bring if necessary:

 Referral Sheet

Anesthesia Medical History Forms

Procedure Forms

Pre-op Instruction Guidelines

Consents (Surgical, Anesthesia, Wisdom Teeth)

Post-op Instruction Guidelines

Along with these available documents, we provide a wealth of training documents and work flow documents to help the office and staff prepare for the surgical day. These include our keys to a smooth day and surgical work flow for the dental assistant to review. All of these in combination help to minimize the front office strain and ensure an easy on-boarding.
