Surgery Forms was created by our team here at Mountain West as a solution to the age long problem of front desk overload. We decided to take it upon ourselves to develop a portal for our offices that would streamline the preoperative process for both the front desk and patient
What we accomplished was a patient portal that allows our clinics to have a secure and direct link to the patient. For all IV sedation and major surgical procedures it is imperative that our doctors receive a comprehensive medical history from the patient. This has always lead to a massive effort by the front desk to send, receive, and file every patients multi-page medical history. Our goal is to bring all the upside of keeping procedures and revenue in-house, while also having the work load for the front desk remain steady.
How Surgery Forms works:
We will link both your clinics email and a google drive, that we create for the office, to our online surgery form.
Each clinics HIPPA compliant surgery form is custom made to account for your specific patients needs.
Once a patient is scheduled they will follow the surgery form link and simply fill out their medical history at their convenience.
This completed form will instantaneously be delivered via email and google drive to your clinic.
This automatically formatted form will be printed off along with our preoperative consent packet and the patient is ready.
This portal also has our easy to follow preoperative and postoperative instructions in case a patient needs to refer to them.
